• What activities would you like to do with your dog? (Companion/pet obedience, competitive obedience, conformation, agility, herding, therapy, search and rescue, other.)

History

• What pets have you previously owned? (Species and breed.)
• How long did you have them?
• Have you ever given away or sold a pet? To whom and why?

Home and Lifestyle

• Do you have children? Names and ages?
• What other pets do you currently own? How long have you had them?
• Where will the puppy be during the day?
• Where and how will the dog be exercised?
• What is your neighborhood like? (City, suburbs, country, other.)
• Do you have a yard? Is it fenced?
• What restrictive dog ordinances are currently in force or proposed in your community? (Breed restrictions, leash laws, mandatory spay/neuter, mandated vaccinations, etc.)
• What will you do with the dog during travel and vacations?
• Who will you be using for veterinary care?

Training

• Who is going to train the dog?
• What kind of training experience do you personally have?
• Do you know what crate training is?
• What do you know about house-training?
• Where is the closest dog training facility to your home that offers classes?
